All prices are plus any applicable taxes. Your selected plan, applicable monthly rate, optional add-on, and enrollment date are recorded at checkout.

1.3 Rate lock. The monthly rate you enroll at is locked for as long as your membership stays active, even if the Business raises its prices later.

1.4 Rejoining. If you rejoin after cancellation, you pay the Business's standard rates in effect when you rejoin, and a new Initial Term begins.

2. Payment Authorization and Billing

2.1 Recurring payment authorization. You authorize the Business, through its payment processors (currently Square and Stripe), to automatically charge the credit or debit card, or debit the bank account, provided at enrollment for:

  1. your first monthly payment on the day you enroll;
  2. your monthly rate, plus any Red Light Upgrade, on each Billing Date until membership ends, except the payment omitted during an approved one-month skip under Section 5.3;
  3. prorated upgrade charges under Section 5.1;
  4. the early termination balance under Section 4.3 and any outstanding amounts owed under Section 2.5.

This authorization stays in effect until your membership ends and all amounts owed are paid. For bank debits, you authorize ACH debits on the same terms and confirm you are an authorized signer on the account.

2.2 Payment method. You agree to keep a valid payment method on file and to update it in writing if it changes. You authorize the Business to use updated card details provided by your card network or bank.

2.3 Failed payments. If a payment fails, the Business will retry it up to three (3) times: 12 hours after the failure, then 1 day after the first retry, then 2 days after the second retry. The Business does not charge a late or declined-payment fee.

2.4 Suspension. While any payment is past due, you may not book or attend sessions. The current Billing Cycle's sessions are issued once the payment succeeds. Sessions already issued keep their original expiration dates during a suspension.

2.5 Default and collections. If any payment remains unpaid 30 days after its Billing Date:

  1. your membership is cancelled and your locked rate ends;
  2. all remaining payments for the Initial Term (if any remain), plus any other unpaid amounts, become immediately due in full;
  3. you authorize the Business to charge that balance to your payment method on file;
  4. the account may be referred to a collection agency or to court, and you agree to pay reasonable collection costs and attorney fees as allowed by Utah law (Section 7.4).

2.6 No refunds. All payments are non-refundable, and no credits are given, except where Section 6.3 or Utah law says otherwise.

3. Sessions and Scheduling

3.1 Issuance. Your plan's sessions are credited to your account on each Billing Date once that month's payment succeeds.

3.2 Expiration. Each session expires 60 days after issuance unless extended under Section 3.3 or Section 5.3. Expired sessions are forfeited and have no cash value. Sessions are not refunded, credited, or exchanged for money, except as stated in Section 3.3.

3.3 Business-caused unavailability. If the Business closes, or the chamber your plan requires is out of service, for 7 or more consecutive days, the expiration date of each affected session is extended by the number of days of unavailability. This is your only remedy for unavailability. It does not apply to closures you were notified of before booking, or to your own scheduling conflicts.

3.4 Booking. Sessions are booked online or through Business staff, subject to availability. The Business does not guarantee any particular time slot.

3.5 Cancellations and no-shows. You may cancel or reschedule a booked session without penalty with at least 24 hours' notice. A session cancelled with less than 24 hours' notice, or missed without notice, is forfeited.

3.6 Late arrival. If you arrive late, your session may be shortened so the next member's session can start on time. A shortened session counts as a full session.

3.7 Daily limits. You may use up to two (2) HBOT sessions per day, at least four (4) hours apart. Staff may further limit same-day sessions for safety or capacity.

3.8 Personal use only. Sessions are for your personal use only. They may not be shared, transferred, gifted, or used by family members or anyone else.

4. Term, Renewal, and Cancellation

4.1 Initial Term. You agree to a six (6) month minimum commitment, which means six (6) consecutive monthly payments starting on your enrollment date. An approved one-month skip (Section 5.3) extends the Initial Term by one Billing Cycle, and a pregnancy pause (Section 5.2) extends it by the pause length. Changing plans does not change it (Section 5.1).

4.2 Automatic renewal. After the Initial Term, your membership automatically renews month to month at your locked rate, and your payment method is charged on each Billing Date until you cancel under Section 4.4.

4.3 Early Cancellation During the Initial Term. You may request early cancellation during the Initial Term by giving notice in writing under Section 4.4 and paying an early termination balance equal to all unpaid monthly HBOT membership payments required to complete six (6) paid Billing Cycles, calculated at your HBOT plan rate in effect on the date of cancellation.

  1. Early termination effective date. If you elect early cancellation, your membership ends on the date the Business receives your written cancellation request and the full early termination balance is paid. The standard 30-day notice period does not apply to an early cancellation under this Section.
  2. Payment authorization. You authorize the Business to charge the full early termination balance to your payment method on file. Any unpaid balance remains due under Section 2.5.
  3. Previously issued sessions. Sessions issued before cancellation remain available for personal use until their applicable expiration dates, including an approved skip extension, subject to the terms of this Agreement. No additional sessions are issued after cancellation.
  4. No refund or credit. The early termination balance is not refundable, and no refund or credit is issued for unused sessions, except as otherwise required by law.
  5. Exceptions. Members qualifying for an exception under Section 4.6 are not required to pay the remaining Initial Term payments.

Submitting a cancellation notice during the Initial Term does not, by itself, release you from your six-paid-payment financial commitment.

4.4 Standard Cancellation Notice. You may submit written notice of cancellation at any time, including during the Initial Term. Cancellation requests must be sent by letter, email, or text message to the Business contact information listed in Section 7.7. Verbal cancellation requests, whether made in person or by telephone, are not valid notice.

  1. Notice effective date. Written notice is effective on the date the Business receives it.
  2. During the Initial Term. If you submit standard cancellation notice during your Initial Term, your membership will end on the later of (a) thirty (30) days after the Business receives your notice, or (b) completion of six paid monthly Billing Cycles, including any extension for an approved skip or pregnancy pause. You remain responsible for all scheduled monthly payments through the membership end date unless you elect early cancellation under Section 4.3 or qualify for an exception under Section 4.6.
  3. After the Initial Term. Once your Initial Term has been completed, you may cancel your month-to-month membership by providing at least thirty (30) days' written notice. Your membership will end thirty (30) days after the Business receives your notice.
  4. Charges during the notice period. Any Billing Date occurring before your membership end date will be charged, except a Billing Date covered by an approved skip under Section 5.3, at your applicable monthly rate. Sessions associated with those payments will be issued and retain their normal expiration dates under Section 3.2. Monthly charges are not prorated for a partial final Billing Cycle.
  5. Cancellation confirmation. The Business will confirm receipt of your cancellation by email or text message, identifying your membership end date and any remaining charges. If you do not receive confirmation within three (3) business days, you should contact the Business to verify receipt. Failure to receive confirmation does not invalidate a cancellation notice actually received by the Business.
  6. No further renewals. Once your membership ends, no additional monthly membership charges will be made, except for outstanding amounts owed under this Agreement.

4.5 After cancellation. Unexpired sessions may be used until they expire. Your locked rate ends when your membership ends (Sections 1.3 and 1.4).

4.6 Exceptions to early cancellation. You may end your membership during the Initial Term without owing the remaining payments only in the following cases, with documentation delivered in writing:

Reason

Documentation required

What happens to issued sessions

Relocation of 80 miles or more from the Business

Proof of new address, such as a lease, closing statement, or utility bill in your name

Voided on the effective date

Death of the Member

Death certificate, provided by the estate or next of kin

Voided

Permanent disability that prevents HBOT use

Signed statement from a licensed physician

Voided

Military deployment or reassignment

Copy of official orders

Voided

Cancellation under this Section takes effect on the date the Business receives the documentation. No further charges are made after that date, and payments already made are not refunded. These are the only exceptions. A new medical condition, a change in schedule, dissatisfaction with results, or a move of less than 80 miles does not qualify.

5. Plan Changes, Pauses, Skips, and Red Light

5.1 Changing plans. You may change to another HBOT plan at any time by request in writing. The change takes effect immediately.

5.2 Pregnancy pause. A pregnant Member may pause the membership by giving the Business a note from a licensed medical provider confirming pregnancy.

  1. The pause starts on the date the Business receives the note.
  2. During the pause, no payments are charged and no sessions are issued. Issued sessions stop counting toward their 60-day expiration.
  3. The pause ends when you give the Business written medical clearance to resume HBOT after delivery. Billing resumes on your next Billing Date after clearance is received.
  4. If no clearance is received within 6 months after the expected due date, the membership resumes on the next Billing Date unless you contact the Business in writing first.
  5. The Initial Term is extended by the length of the pregnancy pause.

5.3 One-month membership skip. An active Member may skip one (1) full Billing Cycle once in any rolling twelve (12)-month period, including during the Initial Term, by requesting the skip in writing before the next Billing Date. There is no minimum advance-notice period. The skip starts on that Billing Date and ends immediately before the following scheduled Billing Date.

5.4 Red Light Upgrade. The Red Light Upgrade is available only with an active HBOT membership.

6. Health, Safety, and Eligibility

6.1 No known contraindications. By signing, you confirm that, to your knowledge, you have no medical condition making HBOT unsafe and that information given to the Business is true and complete.

6.2 Intake and waiver. Before your first session, you must complete the Business's health intake form and informed consent and release of liability, which are incorporated by reference.

6.3 Physician clearance. If you have a medical concern or prior condition, provide written clearance from a licensed physician before your first session. If clearance is denied before your first session, this Agreement is void and your enrollment payment is refunded. After the first session, only the exceptions in Section 4.6 permit early termination without the remaining Initial Term payments.

6.4 Health changes. Before each session, you must tell staff about any change in your health, including new conditions, medications, surgeries, injuries, pregnancy, or ear or sinus problems. Staff may require new physician clearance before you continue.

6.5 Staff discretion. Staff may decline to start a session, or end one early, for your safety or the safety of others. A session that staff decline to start for safety reasons is returned to your account with its original expiration date.

6.6 Wellness service. HBOT and red light therapy at the Business are offered as wellness services. They are not a substitute for medical diagnosis or treatment, and the Business makes no promise of any specific result. Experiences and results vary.

6.7 Facility rules. You agree to follow posted facility and chamber safety rules, including rules about items allowed inside the chamber. Breaking safety rules may result in termination of this Agreement, with the early-cancellation balance in Section 4.3 due.

6.8 Age. Members must be 18 or older. A Member under 18 may enroll only if a parent or legal guardian signs this Agreement. That parent or guardian accepts all of its terms and is fully responsible for all payments. A parent or legal guardian must be present during every HBOT session for a Member under 18.

7. Billing Disputes, Governing Law, and General Terms

7.1 Contact us first. If you believe a charge is wrong, you agree to contact the Business in writing and give it 10 business days to respond before disputing the charge with your bank or card issuer.

7.2 Acknowledgment of authorized charges. You acknowledge that charges made under this Agreement are authorized. This includes charges after you stop using sessions, during the Initial Term, during the 30-day notice period, and for any balance under Sections 2.5 or 4.3. Not using your sessions does not cancel your membership or stop billing.

7.3 Chargebacks. If you dispute an authorized charge, you authorize the Business to give your bank, card issuer, or payment processor a copy of this Agreement and the records related to your account. These may include your online acceptance record (date, time, IP address, and agreement version), your signature and initials, booking and check-in history, and messages between you and the Business. A chargeback does not cancel this Agreement. Any amount reversed through a chargeback that is found to be owed remains due under Section 2.5.

7.4 Attorney fees and costs. In any legal action to enforce or interpret this Agreement, the prevailing party may recover its reasonable attorney fees and court costs from the other party. If the Business must take action to collect amounts you owe and prevails, you also agree to pay its reasonable collection costs, to the extent allowed by Utah law.

7.5 Governing law and venue. This Agreement is governed by the laws of the State of Utah. Any dispute will be filed in the Small Claims Court of Washington County, Utah, when the amount is within its limit, and otherwise in the Fifth Judicial District Court in Washington County, Utah. You consent to that venue.

7.6 Electronic acceptance and signatures. You agree that checking "I have read and agree" at online checkout, your electronic signature, and your initials are each legally binding, the same as a handwritten signature, under the Utah Uniform Electronic Transactions Act. If you enroll online, your checkout acceptance is binding from the moment of your first payment. You may book sessions after online acceptance, but before your first session begins, you must sign in person and complete the health intake and informed consent and release.

7.7 Notices and contact. Written notices to the Business go to:

Notices to you go to the email, phone number, or address provided at enrollment. You must keep your contact details current.

7.8 Communications consent. You agree to receive account, billing, booking, and cancellation messages by email and text message. Message and data rates may apply.

7.9 General terms.

8. Key Terms and Online Acceptance

Your full legal name, date of birth, mailing address, email, mobile phone, enrollment date (Billing Date), selected plan, monthly rate, optional Red Light Upgrade, and payment method are recorded in your checkout enrollment record.

By accepting this Agreement, you acknowledge the following key terms:

  1. I authorize automatic recurring charges on my Billing Date each month until my membership ends (Section 2.1).
  2. I commit to six monthly payments. A skip extends the Initial Term; early cancellation requires paying all remaining payments (Sections 4.1, 4.3, 5.3).
  3. After the Initial Term, my membership renews automatically month to month until I cancel in writing with 30 days' notice (Sections 4.2, 4.4).
  4. Sessions expire 60 days after issuance, subject to the stated extensions, and unused or expired sessions are not refunded or credited (Sections 3.2, 2.6).
  5. Late cancellations (under 24 hours) and no-shows forfeit the session (Section 3.5).
  6. If a payment is 30 days past due, my membership is cancelled, my locked rate ends, and the remaining balance is due and may go to collections (Section 2.5).
  7. My locked rate ends if my membership is cancelled (Sections 1.3–1.4).
  8. I have no known contraindications to HBOT and will report any health changes before each session (Sections 6.1, 6.4).
  9. I will contact the Business before disputing any charge with my bank (Section 7.1).

By checking "I have read and agree" and completing checkout, you confirm that you are at least 18 years old (or are the parent or legal guardian enrolling a minor), that you have read this entire Agreement, and that you agree to all of its terms.
